This FAQ explains how the addendum functions in Axxess Hospice and how due dates, triggers and addendum content are determined.

What is the Non-Covered Items and Services Addendum?

The addendum identifies related diagnoses, unrelated diagnoses, non-covered medications, non-covered DME, non-covered supplies, and other non-covered services or items. It is generated from information currently documented in Axxess Hospice.

What information appears on the addendum?

The addendum includes only information that is active and current at the time it is generated. Think of it as a snapshot in time of the patient record.

Will discontinued diagnoses appear on the addendum?

No. If a diagnosis has been discontinued before the addendum is generated, it will not appear. Only active diagnoses designated as related or unrelated will populate the document.

Will discontinued medications appear on the addendum?

No. Only active medications that are currently identified as covered or non-covered at the time of generation will appear.

Will discontinued DME, supplies, or services appear on the addendum?

No. Only active items and services populate the addendum. Discontinued items do not flow to the document.

When is an addendum required?

An addendum is required upon initial hospice election/admission and whenever changes occur to diagnosis relationships or coverage determinations for medications, DME, supplies, or services.

What starts the five-day addendum timeline?

For the initial addendum, the timeline begins when the patient is admitted to hospice and elects the hospice benefit.

What changes trigger a new addendum requirement?

Any update affecting related diagnoses, unrelated diagnoses, non-covered medications, DME, supplies, or services will trigger a new addendum requirement after the initial addendum is furnished.

How will staff know an addendum is due?

Axxess displays a banner notification indicating that an addendum is due and requires action within the applicable timeframe.

When does the banner disappear?

The banner disappears after the addendum is generated, marked as sent or furnished, and the furnishing action is recorded in the Non-Covered Items table.

Why is the banner still visible?

Refresh the Non-Covered Items table screen. The banner may remain until the page is refreshed. If the banner is associated with an order that is open, sign and complete the associated order.

If I backdate an addendum, will it show historical information?

No. Even when backdated, the addendum reflects only the information that is active on the day it is generated.

What happens if I backdate a medication, supply, or DME start date?

The due-date trigger is based on when the change was entered into the Axxess solution, not the effective start date entered on the item.

What if diagnosis relationships changed in the past?

The addendum will still pull only the diagnoses currently designated as related or unrelated at the time of generation.

Are due dates based on effective dates or entry dates?

Due dates are based on when the change is entered into Axxess because effective dates may differ from when information was recorded.

What should organizations document accurately?

Organizations should ensure diagnosis relationships, non-covered medications, DME, supplies, services, and coverage determinations are maintained accurately and updated promptly.

How does Axxess support FY 2027 requirements?

Beginning October 1, Axxess will provide addendum-generation support, automated reminders, compliance tracking, reporting, audit readiness tools, and workflow enhancements. This feature is inclusive of all active patients and payers.
