Clinical Reports


The Report Center in Axxess Hospice enables organizations to easily retrieve the reporting they need to streamline operations and optimize performance across every area of business.


To access the Report Center, navigate to the Reports tab and select Report Center from the drop-down menu.



Reports are organized into categories based on the data they provide. Categories include Patient Reports, Clinical Reports, Schedule Reports, People Reports, Statistical Reports, Survey Reports, and Billing and Financial Reports.


User permissions determine which reports are accessible to users in the Report Center.


The Learn More link to the right of each report provides a brief description of the report, the parameters that can be used to customize the data, and the information generated by running the report. Click the name of a report to generate it.


Once a report is generated, users can click the Export button in the top right corner to export the data to an Excel spreadsheet for printing and/or saving electronically. The Report Center button on each report screen enables users to navigate directly back to the Report Center.


The following reports are available in the Clinical Reports section of the Report Center.

On-Call Stats


Description: Displays a list of on-call visits performed after hours.

Parameters: Date Range, Branch, Team

Report Includes: Patient Name, MRN, Branch, Team, Task, Status, Employee, Schedule Start, Schedule End

Outstanding Orders by Age


Description: Displays a list of orders waiting to be sent to the physician or pending the physician’s signature. Users can sort and filter by age to categorize orders based on the length of time each order has spent in outstanding status. This report is crucial for tracking physician orders to maintain compliance and streamline billing processes. 

Parameters: Order Age, Branch, Team 

Report Includes: Patient Name, MRN, Branch, Team, Order Age, Order Status, Order Type 


Orders Pending QA Approval


Description: Displays a list of tasks pending QA approval or tasks being returned for addendums. All tasks will display in chronological order from oldest to newest. This report is vital to monitor clinical documentation and ensure compliance.

Parameters: Orders Preventing Billing, Branch, Team

Report Includes: Patient Name, MRN, Branch, Team, Order Type

Top Physicians with Outstanding Orders


Description: Displays a list of outstanding orders that have been sent to the physician. 

Parameters: Branch, Physician, Order Type, Date Range 

Report Includes: Branch, Physician, Patient Name, Order Type, Order Date, Sent Date 

Outstanding Orders by Type


Description: Displays a list of outstanding orders sorted by To Be Sent or Sent to Physician status. 

Parameters: Branch, Order Type, Order Status, Date Range 

Report Includes: Patient Name, Branch, Physician, Order Type, Order Date, Order Status, Approval Date, Sent Date 


Overlapping Frequency Report


Description: Displays a list of overlapping frequencies for an identified effective date. 

Parameters: Effective Date, Branch, Discipline 

Report Includes: Patient Name, Branch, Team, Discipline, Frequency, Start Date, End Date 


Supervisory Visit Report


Description: Displays a list of supervisory visits sorted by discipline and shows the number of days since the last supervisory visit was completed. This report is critical for internal administrative purposes and maintaining compliance.

Parameters: Branch, Team, Employee Name, Task Type, Date

Report Includes: Branch, Team, Patient Name, Employee Name, Task Type, Status, Schedule Date, Days Since Last Supervision


Index
