Axxess Hospice now provides Hospice Outcomes and Patient Evaluation (HOPE) specific visits. These visits ensure regulatory change management.
The RN Initial – HOPE, RN Initial/Comprehensive – HOPE, RN Comprehensive Admission – HOPE, and RN Comprehensive (HUV) – HOPE visits must be enabled in Company Setup.
To enable the visits, navigate to Company Setup under the Admin tab and select Clinical. In the Clinical section, select Enable under the visits and click Save.

Once enabled, the visits will be visible in the Task section. Users with permissions can schedule the visits.

The following restrictions apply until HOPE goes lives:
Scheduled visit dates must be at least the anticipated HOPE go-live date.
Visits cannot be closed until the actual date is at least the expected HOPE go-live date.
The visits comply with the regulatory changes associated with the HOPE changes for both the admission and HOPE Update Visits (HUV) 1 and 2 and Symptom Follow-Up Visits (SFV).
Once visits are enabled, payer fee schedules can be updated based on expected rates and payer setup functionality.
RN Initial – HOPE: Replaces the RN Initial form when HOPE goes live. This visit should be used when the initial visit used to admit a patient is not a full comprehensive visit.
RN Initial/Comprehensive – HOPE: Replaces the RN Initial/Comprehensive form when HOPE goes live. This visit captures all HOPE admission data and includes a comprehensive scrubber to assist clinicians with accurate clinical documentation.
RN Comprehensive Admission – HOPE: Replaces the RN Comprehensive form when HOPE goes live. This visit captures all HOPE admission data in the event that an RN Initial was used to admit a patient instead of the RN Initial/Comprehensive – HOPE form. The visit includes a comprehensive scrubber to assist clinicians with accurate clinical documentation. Clinicians can also use the visit as an SFV visit in the event that an RN Initial – HOPE assessment resulted in a symptom score of two or three requiring symptom follow-up within two business days.
RN Comprehensive (HUV) – HOPE: A new visit introduced when HOPE goes live that captures all HOPE update visit data in both the HUV1 and HUV2 windows. This visit includes a comprehensive scrubber for HUV data. It can be used as an SFV visit when one is required following a previous assessment that resulted in a symptom score of two or three requiring symptom follow-up within two business days.
Symptom Follow-Up Visits: The RN Comprehensive Admission – HOPE and RN Comprehensive (HUV) – HOPE visits can be used as symptom follow-up visits if the visit is being completed within an SFV window. However, there will be no introduction of a new nursing visit for symptom follow-up visits. Rather, all of our existing care visits can function as symptom follow-up visits. The recommended visits for this purpose are the Skilled Nurse Visit, Skilled Nurse PRN Visit, LPN/LVN Visit or LPN/LVN PRN Visit.
