The Axxess Hospice Hope Center is a powerful operational tool that helps organizations ensure compliance with HOPE regulations and provides clear visibility into the data being submitted onto the HOPE record. The HOPE Center provides a comprehensive view of a patient’s HOPE timeline and includes a symptom impact tracker, and interactive HOPE forms table, and a detailed record view with a scrubber. By enabling users to track required assessments and the status of each required CMS record in a centralized and intuitive interface, this enhancement streamlines operations and facilitates compliance.

Symptom Impact Tracker


This tracker enables users to easily understand a patient’s progress through their HOPE requirements and identify any pending follow-up actions. It is generated automatically upon a patient’s admission within the HOPE regulatory timeline.


The tracker establishes and tracks three critical timelines for symptom assessments and symptom follow-ups:


  • Admission: First 5 days of care after admission.

  • HOPE Update Visit 1 (HUV 1): Days 6-15 of services.

  • HOPE Update Visit 2 (HUV 2): Days 16-30 of services.


For each timeline, the tracker indicates the status of the initial symptom assessment as completed or not completed.


It also tracks the status of the symptom follow-up assessment if required based on the severity of the initial assessment:


  • Required but not complete: A follow-up is needed but hasn’t been done.

  • Completed: The follow-up assessment has been completed.

  • Not required: A follow-up was not necessary based on the initial assessment.


The relevant timeline window will be highlighted as the patient progresses through their care, providing a clear visual indicator of the current stage.

Interactive HOPE Forms Table


Located below the symptom tracker, this interactive table displays the four required HOPE record types for CMS submission:


  • Admission: Admission type record.

  • HOPE Update Visit 1: HOPE Update Visit 1 type record.

  • HOPE Update Visit 2: HOPE Update Visit 2 type record.

  • Discharge: Discharge type record.


The table displays the current status of each record type:


  • Generated: The record has been automatically created when the patient enters the corresponding period of care (ADM upon admission, HUV 1 on day 6, HUV 2 on day 16, DC upon completion of a discharge or death note.)

  • Approved: The record has been reviewed and approved.

  • Submitted: The record has been successfully submitted to CMS.


Detailed Record View with Scrubber


Similar to the HIS export manager, a view details option will be available to the right of each HOPE record in the table.


View Details enables users to view each section, question and answer for the selected CMS form.


This detailed view will also include a scrubber to identify any inconsistencies or errors before submission, ensuring data integrity and compliance.
