Axxess Home Health now provides a Diagnosis History screen that users can access in each patient’s chart to view all diagnosis changes recorded for the patient. This enhancement streamlines operations by providing a centralized view of all diagnosis updates, reducing manual tracking, and ensures compliance by maintaining a clear audit trail of diagnosis changes across clinical documentation.
To view a patient’s diagnosis history, open the patient’s chart, click Edit Patient, and navigate to the Clinical/Diagnoses tab. In the Patient Diagnosis section, click View Diagnosis History.

When View Diagnosis History is selected, the Diagnosis History window will open and display a complete list of diagnosis updates for the patient with the source of the update, the user who made the update, and the date the update was made. For entries tied to an assessment or change in focus, the user will default to the clinician assigned to the task and the date will default to the scheduled date of the task.
To view the list of diagnoses as they appeared at the time of the update, click the history icon under Actions. This opens a snapshot of the patient’s diagnoses at the moment the update occurred, enabling you to see exactly what was documented then.
Users with permission to add, edit and delete diagnoses will have access to additional functionality.
For active patients, users will now see an + Add Diagnosis button on the Clinical/Diagnoses tab of the patient’s chart, enabling manual diagnosis updates as needed.
If a diagnosis is documented manually in the patient chart, users with permission to delete diagnoses can delete that entry in the Diagnosis History window by selecting the trash can icon under Actions.
Deleting a manual entry will automatically revert the patient’s diagnoses back to those listed in the previous snapshot.
