How do I add a new episode?

  1. Hover over the Schedule tab and select Schedule Center.
  2. On the left side of the Schedule Center, use the filters and/or search feature to find the patient you want to add a new episode for and select the patient when found.
  3. Along the top of the Schedule Center, select New Episode.
  4. A new window will appear, allowing you to add and verify the new episode information. Click Save.

How do I delete an episode?

  1. Hover over the Schedule tab and select Schedule Center.
  2. On the left, use the filter or search options to find the patient you want to add a new episode for and select the patient when found
  3. In the patient’s chart, go to Actions and choose Manage Episode
  4. A pop-up window will appear with a box to Inactivate Episode is there, place a check mark in this box
  5. Click Save
  6. The system will refresh and the episode will disappear

How do I edit episode information?

  1. Go to the Schedule tab
  2. Select the Schedule Center
  3. On the left, use the filter or search options to find the patient you want to edit episode information for and select the patient when found
  4. In the patient’s chart, go to Actions and choose Manage Episode
  5. A pop-up window will appear with all the current episode information. Make changes as you see fit
  6. Click Save

Can I schedule a full certification period without clicking on every day?

If you need to schedule for a full certification every day, follow the steps below.

  1. In the Schedule Center for the specified patient, click on Show Scheduler
  2. Select Daily Outlier
  3. Select Task
  4. Select the User the certification period will be assigned to
  5. Select the date range
  6. Save

How do I reassign a nurse’s entire schedule?

  2. Select Reassign Schedule
  3. Fill out the information
  4. Click Reassign. The system will check all patient charts for all visits not yet completed by the nurse you want to remove and reassign them to the new user.

How do I reassign scheduled tasks/visits after a user has been deleted?

How do I view a list of Upcoming Recertifications?

You can only do this with special permissions. However, if you have those permissions, click on Upcoming Recerts from your Dashboard. You can also find Upcoming Recerts under the View tab.

How do I get a list of Past Due Recertification?

You can only do this with special permissions. However, if you have those permissions, click on Past Due Recerts from your Dashboard. You can also find Past Due Recerts under the View tab.

How do I discharge a patient?

You will first need to schedule a discharge OASIS Assessment or a discharge summary depending on the discharge circumstances. Then, complete the OASIS and change the patient status to discharged within the patient’s chart.

How do I re-admit a previously discharged patient?

  1. Go to the Patients tab
  2. Choose Patient Charts
  3. On the left, filter to Discharged Patients to find the patient you want to re-admit and select the patient when found
  4. Click the Re-Admit button on the right of the patient’s chart
  5. Verify the information
  6. Click Re-Admit

How do I view the records for discharged patients?

  1. Go to the Patients tab
  2. Choose Patient Charts
  3. On the left, filter to Discharged Patients to find the patient you want
  4. Select the patient when found

How do I cancel an already submitted OASIS assessment?

  1. Go to the View tab
  2. Click Exported OASIS
  3. Find the OASIS you need to cancel
  4. Click Generate Cancel on the right side of the screen

How do I document a Physician Face-to-Face Encounter in the Axxess Home Health solution?

  1. Upon admission of a patient, the Home Health solution will automatically generate a Face-to-Face Encounter to be sent to the physician for completion.
  2. You can also manually generate a Face-to-Face Encounter by selecting New under the Create or Admin tab. Select Physician Face-to-Face Encounter and complete the form.

How do I edit a Patient’s Demographics once the patient is in a Pending Admission status?

Note: The user must have the correct permissions to edit Demographics. (Clerical: Add/Edit/Delete Patients – Manage Patients)

  1. Go to “Patient’s” on the toolbar
  2. From the sub-menu, select “Pending Admissions”
  3. Find appropriate Patient and select “Edit” on the far right.
