Sandata Missouri Memo Requirement

Axxess Home Health users in Missouri who use Sandata as their EVV aggregator can now enter a memo on all EVV visits associated with Division of Developmental Disabilities (DDD) services under Provider Type 85 – Community-Based Developmental Disabilities.


This enhancement facilitates compliance with Sandata memo requirements, preventing claim delays and reducing administrative rework.

What’s New


When a visit is mapped to require a memo, users will now be required to enter a memo between 25 and 1,024 characters. This requirement applies regardless of the reason code selected. The visit’s memo field should contain a meaningful description of the individual’s activities and how they were supported during the delivery of the service. Some providers use the memo field as their daily progress note documentation.


Special Rule for Reason Code 280


For Reason Code 280 – Accrued Minutes Visit, providers may use the default memo:


“This is an accrued minutes visit.”


This default memo meets the memo requirement and can be used without additional text.
