To ensure your organization is prepared to collect the most accurate OASIS data, Axxess has created a list of frequently asked questions to help navigate OASIS changes in Axxess Home Health.
OASIS-E2 introduces the removal, addition and modification of certain assessment items. Changes between OASIS-E1 and OASIS-E2 include:
A0810 Sex replaced M0069 Gender in all time points.
A1110 Patient’s Language was added to the Resumption of Care assessment.
A1255 Transportation replaced A1250 Transportation in the Start of Care and Resumption of Care assessments.
B0200 Hearing was added to the Resumption of Care assessment.
B1000 Vision was added to the Resumption of Care assessment.
D0150 Patient Mood Interview (PHQ 2-9) added the dash symbol ( – ) as an accepted value for A2 – I2.
O0350 Patient’s COVID-19 vaccination status was removed from the Transfer, Transfer Discharge, Discharge and Death at Home assessments.
Per the Centers for Medicare and Medicaid Services (CMS), assessments with an M0090 date of April 1, 2026, or later are required to use OASIS-E2. Assessments with an M0090 date of March 31, 2026, or earlier will continue to use OASIS-E1.
The M0090 date reflects the date all information from other disciplines and the physician has been gathered. The actual documentation of findings is not included in the five-day window allowed for the completion of OASIS items.
OASIS-E1 must be used for patient assessments (SOC/ROC, recertifications, transfers and discharges) completed between March 26 and March 31. If all information is gathered from physicians, other team members and caregivers within the limited (five-day) timeframe for completion, the M0090 date will be March 31, 2026.
OASIS-E2 will be used for patient assessments (SOC/ROC, recertifications, transfers and discharges) completed with an M0090 date of April 1, 2026, or later. Using an incorrect assessment or entering the wrong M0090 data will prevent the assessment from being completed.
Organizations with the Pending Admissions Management setting enabled in Company Setup in Axxess Home Health will need to review their Pending Admissions for OASIS-E1 Start of Care tasks. If an OASIS-E1 version is scheduled, it must be updated to the OASIS-E2 version before the patient can be admitted. To update the task:
1. Delete the currently scheduled OASIS.
2. Reschedule the Start of Care using the correct OASIS version.
Once the correct version is scheduled, the admission can proceed without any interruption to the pending admissions feature.
Axxess Home Health is updating the Medicare fee schedule for existing organizations with default rates and codes assigned. Users can edit their Medicare fee schedule if their rates or codes differ from the default values. For all other insurance payers, the organization will have to enter the rates independently or load rates from another payer with the rates defined.
In Axxess Home Health, organizations will need to add OASIS-E2 user pay rates in their user profile. Once rates have been added for a single user, rates can be copied from one user to another with the same rate requirements.
The Load Previous functionality will only flow from the same assessments, for example from OASIS-E2 to OASIS-E2. Therefore, the Load Previous function will not be available for the first completed OASIS-E2 assessment.
