To help ensure visit notes reflect the most up-to-date care plan, Axxess Home Care now detects when a care plan has been updated since a visit note was last saved and prompts users to review and update the note as needed. This enhancement reduces the risk of documenting against outdated care plan information, supports accurate clinical records and ensures caregivers have access to the most current care instructions during client visits.
When a user edits a visit note after the associated care plan has been updated, the following message will appear: The associated care plan has been updated. Would you like to update this note with the updated care plan?
When the user selects Yes, the visit note is then updated with the latest care plan information. Information such as service frequency, safety precautions, permitted activities and daily living tasks, is updated to reflect the current care plan.
When the user selects No, the visit note remains unchanged and continues to use the care plan information previously loaded into the note. Any documentation completed afterward may not reflect the most recent care plan revisions.
Please note: This prompt only appears when the associated care plan has been modified since the visit note was last saved.
