Organizations in Missouri using Axxess Home Care with Sandata as their EVV aggregator can now add a memo to all EVV visits associated with Division of Developmental Disabilities (DDD) services under Provider Type 85 – Community‑Based Developmental Disabilities. This enhancement ensures compliance with Missouri Sandata memo requirements, preventing claim delays and rework associated with incomplete files.
To enter a memo in Axxess Home Care, navigate to the EVV Details screen, select an applicable reason code from the Reason Code drop-down menu and navigate to the Memo field to enter the memo. Note: Regardless of the reason code entered, a memo is required.

Memos must include at least 25 characters and no more than 1,024, and they should clearly describe the individual’s activities and the support provided during the service. Many providers also use the memo field as their daily progress note documentation.
For Reason Code 280 (Accrued Minute Visit) users can enter the default description This is an accrued minutes visit in the Memo field.
This description meets the memo requirement and can be used without additional text.
