Axxess Palliative Care has enhanced the Functional section of the Physical Assessment for initial and provider visits, creating a more comprehensive workflow for documenting Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). This enhancement enables users to complete the screenings directly within the solution, streamlining clinical documentation, supporting quality measure requirements, and improving workflow efficiency.

Lawton and Brody IADL Scale

Users can now complete the Lawton and Brody (IADL) Scale directly within the Physical Assessment. The assessment includes the following categories:

• Ability to Use Telephone

• Shopping

• Food Preparation

• Housekeeping

• Laundry

• Mode of Transportation

• Responsibility for Own Medication

• Ability to Handle Finances

Users can select a single response for each category, and the solution automatically calculates and displays the total score (0-8) along with the corresponding functional status. Scores range from 0, indicating low function and high dependence, to 8, indicating high function and independence.

Improved ADL Assessment Workflow

The Functional section has also been reorganized to streamline ADL documentation and support quality measure requirements. Under Abnormalities Found, a new ADL and IADL Assessment option is available with the following choices:

• ADL Assessment

• IADL Assessment

• Assessment not performed due to documented advanced-stage dementia and limited caregiver knowledge.

The ADL Assessment now includes the following functional areas:

• Ambulation

• Transfer

• Eating

• Dressing

• Bathing

• Grooming

• Toileting

For each category, users can select one of four assistance levels: Independent, Minimal, Moderate or Complete.
