Axxess Palliative Care now provides a Tobacco Screening and Tobacco Cessation Intervention task, enabling clinicians to document tobacco use details, cessation readiness and intervention techniques.
The new Tobacco Screening replaces the previous manual entry process for tobacco-related fields on the Health History tab in each patient’s chart. These fields are now read-only and automatically populated with data from the Tobacco Screening. Information entered in the Tobacco Screening also flows into the Health History section of visit notes and the new Tobacco Cessation Intervention task, streamlining documentation, ensuring follow-up and supporting billing for these services.
To document a Tobacco Screening for a patient, navigate to the Patients tab and select Screenings. On the Screenings page, navigate to the Tobacco tab and click Add Tobacco Screening.

On the Tobacco Screening page, select the appropriate responses under Current Smoking Status and Historical Smoking status. The system will require start and end dates when appropriate. Once the form is saved, the results will appear in the table on the Tobacco tab.
The fields on the Health History tab in each patient’s chart remain visible but are now read-only. They now automatically populate with the values entered in the Tobacco Screening.

To update these fields from the patient chart, click Add Tobacco Screening to open the Tobacco Screening.
The results of the Tobacco Screening will appear in the Tobacco Cessation Intervention task, which provides fields to document additional details about the patient’s tobacco use and interventions used.
