At the 2025 Axxess Growth, Innovation and Leadership Experience (AGILE), Liberty Home Care and Hospice received the Quality Standards of Excellence Award, recognizing their commitment to delivering the highest quality of care to patients.

In an interview with Axxess, Donna Turlington, RN, BS, HCM, CCM, President of Liberty Home Care, Hospice and Palliative Care, shared how this combination of dedicated people and innovative technology enabled their organization to achieve measurable improvements and sustainable growth and excellence.
Achievements in Quality and Growth
Highlighting the organization’s recent quality achievements, Turlington reported that several home health locations raised their quality ratings.
“Ten of our home health locations have achieved a star rating of 3.5 or higher,” said Turlington. “In many cases, we increased the ratings by a full point, moving from two to three stars and from 3.5 to four stars.”
She also emphasized the strong performance of the organization’s hospice locations, noting their improved quality scores.
“Our hospice locations currently hold a four-star rating and have achieved a Hospice Care Index (HCI) rating of 10,” said Turlington.
In addition to their improved quality scores, the hospice locations also saw increased census and revenue growth.
“We have experienced significant census and revenue growth for our hospice services, with a 48% increase in census and a 52% increase in revenue over the past year,” said Turlington.

Leveraging Technology for Success
Reflecting on how Liberty’s home health and hospice locations achieved significant improvements in quality metrics, Turlington highlighted the ways the Axxess ecosystem has played a key role in their success.
“We are continuously identifying ways to leverage the solution to ensure quality and grow our business,” she said.
Turlington emphasized that tools like Axxess Home Health’s PDGM Dashboard and the Quality Assurance (QA) Center have been instrumental in keeping their home health locations compliant, as well as supported them in patient outcome monitoring and driving revenue growth.
Turlington identified the built-in scrubber and predictive dashboards in Axxess Hospice as essential hospice tools, noting that these features helped strengthen documentation processes and compliance by ensuring a thorough review before submission to CMS.
The many Axxess Business Intelligence reporting tools were also crucial to their quality and growth successes. These tools provided pivotal visibility into their overall operational performance and metrics.
“We use these tools to continuously evaluate and assess our operations, enabling us to address issues and pivot as needed,” said Turlington.
