The Guiding an Improved Dementia Experience (GUIDE) Model is a Centers for Medicare and Medicaid Services (CMS) initiative designed to test whether standardized dementia care programs (DCPs), providing support for dementia patients and their unpaid caregivers, improve patient outcomes and lowers costs.

Palliative care providers are uniquely positioned to contract with GUIDE program participants. With a focus on relieving symptoms and stress for patients, their loved ones and caregivers, palliative care providers can leverage their knowledge, experience and existing practices to contractually provide some or all GUIDE care for providers selected to participate in the GUIDE Model.

Dual eligibility for Medicare and Medicaid
Performance-based adjustment (PBA): Increase or decrease monthly DCMP based on the previous performance year.
