Hospice agencies are required to complete documentation that authorizes their CAHPS Hospice Survey vendor to collect and submit survey data to CMS. Survey vendors cannot submit CAHPS Survey data on a hospice agency’s behalf without their documented authorization to do so.

Complete the Data Warehouse Access Form and one of the Survey Vendor Authorization Forms, and submit the complete forms to the CAHPS Hospice Survey Team identified on the forms:
A hospice must have access to the CAHPS Hospice Survey Data Warehouse in order to monitor the data submitted by its authorized survey vendor. Complete this form to get access to the CAHPS Hospice Data Warehouse.
Complete this form if authorizing a CAHPS Hospice Survey vendor for the first time.
