Referrals


Axxess Central enables users to create and edit referrals directly from the Central Intake tab, streamlining the referral intake process for organizations with multiple service lines.

Add a New Referral


To create, view or edit referrals, navigate to the Central Intake tab and select Referrals from the drop-down menu.

On the Referral page, select New in the top-right corner.


On the Add Referral screen, click the New Referral button and select the appropriate service line for the referral.


On the New Patient Referral screen, enter the required information and select Add Referral.


On the Referral Information screen, enter the patient’s or client’s information.


Note: The home health, hospice, home care and palliative care referral forms in Axxess Central are identical to their respective solution referral forms.


On the home health, hospice and palliative care referral forms, users can check a patient’s Medicare eligibility using the Medicare Eligibility Check feature within the Payer Information section. In this section, a red X icon will display next to fields that have not been entered but are required to complete a Medicare eligibility check.


When all the required fields are entered, a Check Eligibility button will appear.


Once all required fields on the referral form are completed, select the Create Referral button at the bottom of the form.


When saved, the Referrals screen will appear and a confirmation message will display, indicating that the referral was saved successfully.


Pending referrals will be housed on the Referrals screen. Using the filters at the top of the page, users can search for referrals by name, agency or branch.


Using the Actions column, referrals can be edited, transferred to another agency or branch, non-admitted, converted to pending or deleted.


Edit a Referral


To edit a referral, select the edit icon under the Actions column and make any necessary updates. When complete, click the Save Inquiry button to apply the changes. Select Cancel to return to the list without saving.

Delete a Referral


To delete a referral, select the ellipsis under the Actions column and click Delete. A confirmation message will appear, prompting the user to confirm the action before proceeding with the deletion.


Convert Referrals to Pending (Not Applicable for Home Care Referrals)


To convert a home health, hospice or palliative care referral to pending status, navigate to the desired referral and select the ellipsis under the Actions column. Select Convert to Pending from the drop-down menu.


A referral can also be converted to pending status immediately after entering or editing information in the referral form by clicking the Convert to Pending button at the bottom of the form.


Once all required information has been entered, the referral will be converted to a pending patient. All referral information entered will flow to the patient chart.

Add Referral Documents


Users can access any documents attached to a referral from the Referrals screen. Depending on their permissions, users can view, edit or delete referral documents.


To access referral documents, navigate to the paper clip icon under the Actions column.


On the pop-up screen, select Add Documents to add new documents to the referral. When complete, click Save Documents to attach the documents to the referral.


To view or edit the documents, select the edit icon under the Actions column. To delete documents, select the trash can icon.

Export Referrals


On the Referrals screen, users with the appropriate permissions can export the list of referrals from all service lines into a single Excel file. To export the list, click the Export Excel button.
