Axxess Central enables users to seamlessly transfer patients from one provider or system to another, ensuring continuity of care as patient needs evolve.

To transfer a patient, navigate to the Central Intake tab and select Referrals.

Use the filters at the top of the screen to locate the desired patient.

Click the refer icon under Actions next to the desired patient.

Complete the required fields in the window that appears and click Refer. The Non-Admit Current Patient or Referral option determines whether the patient or referral should remain in the system. The Refer To field determines where the patient will be transferred. The Branch field determines the specific branch within the selected provider network where the patient will be transferred.

Once Refer is selected, the Referral Source screen will appear, where additional patient details such as demographic information, tags, payers, clinical data, and referral comments can be reviewed and updated.

Once all required fields are completed, click Create Referral to transfer the patient to the new provider or system.
