Axxess’ Initiatives for the Transition to HOPE

As hospice care regulations evolve, Hospice Outcomes and Patient Evaluation (HOPE) will become the new assessment tool for ensuring compliance while providing support. Axxess is at the forefront of ensuring hospice organizations have the tools and resources they need to navigate the HOPE changes, which will take effect October 1.

Christina Andrews, Senior Director of Professional Services at Axxess, and Raianne Melton, Director of Clinical Services at Axxess, highlighted the importance of providing practical, actionable guidance alongside a deeper understanding of the concept of HOPE and its implementation.

Commitment to Thought Leadership and Solutions

Axxess is focused on supporting clients’ successful transition to HOPE.

Axxess’ approach goes beyond discussions and focuses on actionable items and practical solutions.

“We’re going to leverage those relationships to have webinars regarding HOPE,” said Melton. “It’s not going to be just about an overview of HOPE. We understand that our clients are busy running their businesses. They need actionable items.”

One of the initiatives is the HOPE workshop at AGILE (Axxess Growth, Innovation and Leadership Experience), which will be held May 5-7 in Dallas. The workshop is designed to incorporate tabletop exercises that enable participants to apply what they have learned in real-time. Industry experts guide attendees through every aspect of HOPE, from item selection to scheduling and submission.

The HOPE Toolbox: Resources for Success

Axxess is providing exclusive tools to support hospice organizations in their transition to HOPE. These resources include:

  • The Road to Readiness: A manager’s guide to training, complete with checklists to ensure stakeholders receive proper training.

  • HOPE Ahead: Proceed With Confidence: An eBook providing an overview of regulatory processes.

  • The ABCs of HOPE: A detailed item-by-item instruction manual with best practices and tips.

  • Discipline-Specific Competencies: Ensures staff are well-trained and knowledgeable.

  • Intake Data Entry Sheet and Checklist: Streamlines the admissions process by capturing essential administrative details.

“The HOPE toolbox is comprehensive,” said Andrews. “It’s going to continue to expand on what our [Axxess] care at home experts are going to need to ensure great success.”

These tools will be exclusively available to HOPE workshop attendees at AGILE.
