When running a census report, I noticed a patient’s name appearing on the report who had been discharged more than a year ago. So I investigated.
I went to the patient’s chart in Axxess. I clicked on “Admission Periods” and the window that opened showed the Patient had three admission periods listed.
The 1st one (the oldest) had both an admission date and discharge date. Everything with it looked fine.
However, the 2nd in the list showed only an admission date. No discharge date was listed. So I thought I had found the reason as to why the patient was still showing up on my census report. Simply adding the discharge date would fix my problem, or so I thought.
I clicked to “Edit” that admission period, and a new window with the patients information appeared. I quickly located the space for the discharge date where no data had been entered. I entered the date of the patient’s discharge, which is when I received an error message which read: "Admission period (SOC & DC date) is not in the valid date range."
I made several attempts to enter the date, which I knew to be correct, and I kept receiving the same error message.
The problem, I believe, lies with the 3rd admission period in the list. It showed an admit date the same as the 2nd admission period, AND it showed a discharge date the same as the one I was trying to enter for the 2nd admission period. Moreover, there are no episodes associated with the 3rd admission period in the list.
I clicked to “edit” the 3rd admission period, and the window which opened only showed an ICD diagnosis code. No other information than that, just the diagnosis.
I would like to delete that 3rd admission period, because it was clearly entered by mistake. The patient only had two admission periods.
I’ve tried everything I can think of to remove that 3rd admission period, and to enter the discharge date into the 2nd admission period, but to no avail. Any advice would be appreciated.
